Design Strategy:

In addition to an email link, a contact page with submittable fields allows a user to send a response without having to have email preferences set up in the browser, and also allows the user to send a response from a computer other than his/her own. This form has both required and non-required fields, ensuring that the critical information must be filled in and sent along with the comments.
